Youth Alive Trust
TERMS AND CONDITIONS OF ENROLLMENT
By enrolling my child or a child I am responsible for, I agree to the following:
1. Programme Operating Hours
a. Programmes run at the times advertised.
b. Staff are not responsible for children any time outside 10 minutes before or 15 minutes after the advertised hours.
c. If my child is still in the Trust's care 15 minutes after the advertised time and the responsible member of staff had not had any phone call explaining the reason, emergency contacts will be contacted, and I may be liable to pay for extra care.
d. It is my responsibility to inform the programmes manager or programme coordinator if my child will arrive later or leave earlier than the times advertised.
2. Cancellations and Refunds
a. If my child is unable to attend the programme on the day, I am responsible for informing Youth Alive Trust of their absence.b. Notice of at least 24 hours where possible should be given when cancelling your child’s/children’s booking in order for refund policy to apply.c. In the event of withdrawing my child I will be entitled to a refund of what I have paid less 15% administrative/booking fee.
3. Health and Safety
a. I acknowledge and bear the responsibility and risk to my child participating in programme activities.b. I have explained to my child the need to follow safety instructions, remain in areas designated by staff, and refrain from behaviour that could cause injury.c. I acknowledge that Youth Alive Trust staff will do their utmost best to ensure the safety of young people participating in programme activities at all times, yet recognise that accidents and injuries can still happen.d. I give permission for staff with a current first aid qualification to give any necessary medical treatment in the event of an injury.e. Any further medical attention will be at my own expense. f. I give permission for my child to be taken on offsite excursions, which may include travelling on foot or making use of different modes of motorised transport to get to the location.g. I give permission for my child to be transported in accordance with Youth Alive Trust ‘Policies and Procedures’.h. I understand that I need to sign my child/children in and out of the programmes, unless they are 12 years old or older, and have been added as an authorised signatory to my user profile.
i. I will notify staff if someone not currently listed as an authorised signatory on my user profile is to pick my child up.j. In the event of an earthquake or fire everyone present will gather at the Seafield-building car park on Beresford Street, or the open space in front of Grace Vineyard on Seaview Road.k. In the event of a building lock-down in the interest of the safety of children and staff, I will follow all the reasonable instructions of Youth Alive Trust Management in accordance with the Trust’s Policies and Procedures.
4. Property
a. Youth Alive Trust takes no responsibility for loss or damage to any child's personal possessions and equipment, including electronic devices.b. Lost property will be kept at reception for a period of 3 weeks; any lost property that has not been claimed after 3 weeks will be donated.c. I acknowledge that any willful damage to equipment or property that has been caused by my child will incur an additional charge.
5. Privacy and Access to Information
a. I acknowledge that all information supplied in this booking application will be treated with confidentiality and kept in a secure database, in accordance with the Privacy Act 2020.b. Access to the supplied contents will be limited to authorised staff of Youth Alive Trust.c. Youth Alive Trust reserves the right to limit confidentiality where there is reasonable concerns about the health and safety of a child.d. I have the right to view any information on my child/children at any time, and acknowledge that this information may also be viewed by an assessor appointed by the Ministry of Social Development.e. Photos and videos taken at Youth Alive Trust Programmes may be used for Youth Alive Trust publicity purposes.f. I will inform the programmes manager in writing if I do not want my child/children’s photos and /or name displayed on any publicity material.g. I have the right to view the Youth Alive Trust Policies and Procedures at any time.
